Definitions:
Teeth Whitening:
As we age and the teeth are exposed to foods and drinks that stain, our teeth will darken. Tooth whitening creates a more esthetic appearance by reversing the discoloration of tooth enamel. Not everyone will have the same results. Some will whiten more and faster than others. To determine if a patient is a good candidate, a careful examination and x-rays will help us find the cause of the discoloration. Certain factors which can contribute to the discoloration include: antibiotic staining, excessive fluoride, trauma, pulp degeneration, filling materials, genetic and acquired illnesses, aging and consumption of tobacco, coffee, tea and red wine.
Whitening is done using one of two methods.
- You can wear a custom made tray in which the whitening gel is placed and worn either over night or for 3 hours daily. Both of these take two to three weeks for your teeth to whiten.
- The other method is done at A Brilliant Smile Office, and only takes about one hour.
The in office procedure uses same methods as seen on the “Extreme Makeover” TV show.
While at A Brilliant Smile Office in Woodland Hills, we place a protective drape over your lips and gums and place the whitening gel on your teeth. The gel is placed and activated with the light for three or four 15-minute sessions. At the end of the appointment you will be given custom made trays to take home for future touch ups. Although neither procedure produces a permanent effect, annual touch ups of only a few days can maintain the original whiten result.
Porcelain Veneers:
Porcelain veneers are thin shells of ceramic that bond directly to the front surfaces of the teeth. They are an ideal choice for improving your smile and have become increasingly popular due to their simplicity and versatility.
Placing custom veneers requires a high degree of technical skill as well as attention to cosmetic detail. We place veneers routinely, and design each case individually to match and enhance the characteristics of each patient’s smile. When the veneer is bonded to the teeth, the ultra-thin porcelain veneers are virtually undetectable and highly resistant to coffee, tea, or even cigarette stains. For strength and appearance, their resemblance to healthy, white tooth enamel is unsurpassed by other restorative options. With proper care, porcelain veneers will brighten your smile for well over a decade. Dr Shirin will ensure that your veneers are crafted form the highest quality porcelains and are bonded with the most advanced and proven materials available. Refraining form using your veneers as tools to open and cut things will prolong their life, and if accidental breakage or damage occurs, it is usually possible to replace only the veneers involved.
Fillings:
Fillings are done to remove decay, and replace the affected tooth structure. It is called a filling because new material fills a hole that the decay left. Now days, most teeth are treated with bonded tooth colored composite fillings. If caught early enough, cavities can be treated easily and painlessly. If not treated, decay can lead to tooth pain and/or infection, and the tooth would need root canal treatment or extraction.
Sealants:
This is used to fill in narrow grooves in a tooth that cannot be adequately cleaned by brushing. In some cases, the tooth structure has fine grooves or pits, which accumulate plaque, not because the person doesn’t brush, but because they’re too narrow to allow even one bristle into them. These will develop cavities over rime, and you don’t want that. So the dentist will brush on a coating that seals the grooves and pits, making it possible to brush off all the plaque and keep your teeth healthy.
Scaling and Root Planning:
Scaling is what is done below the gum line to remove plaque and bacteria. Root planning smoothes the root surfaces so the gums reattach to the root. Usually, this will be accomplished with the use of local anesthetics so your treatment is completely comfortable.
Antibiotics:
Periodontal disease is a bacterial disease, and the key to controlling it is the effective reduction of harmful bacteria. In addition to scaling and root planning, the use of antibiotics delivered directly below the gum line at the site of infection may be used. An antibacterial mouth rinse may also be prescribed to control the harmful bacteria.
Dentures:
There are different types of dentures, but they share their common function. They replace teeth that have become loose or been lost due to bone loss. When bone loss around the roots of teeth is great enough to loosed them, or let them out its time for dentures. Relax. Nobody likes loosing their natural teeth, but you can still eat and talk regularly.
The entire mouth is examined and determination is made as to which teeth will have to be removed, and which will remain. The loose teeth are the extracted. Dentures are fitted to go over or round whatever teeth remain in the mouth, depending on the type. There is an adjustment period after dentures are placed in the mouth, and it can take some getting used to. But once accustomed to the dentures, all the normal functionality and appearance return and one just carries on as usual. Often implants can be used to further stabilize the dentures.
Implants:
A dental implant is an option to replace a missing tooth. In this procedure, a small titanium shaft is surgically implanted into the bone and allowed to set. The bone grows around it forming a tight connection, which additionally slows or stops the bone loss that occurs when the root of a natural tooth is missing. Once the implant is firmly set in the mouth, the dentist then works to attach the replacement tooth onto the top of the shaft. This permanent solution has the advantages over bridge work that it does not stress the surrounding teeth for support, and, should the tooth wear out, another can simply be replaced on the shaft.
Root Canal Treatment:
Root canal treatment (also referred to as endodontic therapy) is made necessary when a cavity is allowed, through neglect, to reach all the way to this pulp. (Regular cleaning and check ups prevent and detect problems early). Sometimes deep restorations or trauma to a tooth may cause the nerve to be damaged to the point it needs root canal therapy. Once this occurs the pulp becomes infected, and can even extend through the root tip and begin to eat away at the surrounding bone (this is an abscess). By the time the pulp is infected it must be treated, and cannot heal on its own. It can even weaken the entire immune system, this is dangerous, not to mention very painful. Symptoms that the pulp has become infected may include sensitivity to hot, cold or sweets, pain, swelling, pain to biting or pressure, and a bad taste in the mouth. Sometimes, however, no symptoms are apparent and the person is unaware of any problem until a check up.
A root canal is then performed to clean out the infected tooth pulp, and disinfect the canals of the tooth; the only other treatment would be to extract the tooth. Once the infection is resolved, the canal(s) are filled in to prevent any further infection. Usually a core build-up and crown is recommended for restoring a tooth that has had root canal therapy.
